- 📌 Basic Application & Legal Agreements (Pages 1-2): This form gathers all the important information we need about your cadet. It includes the necessary parental consent and media release permissions.
- Report of Medical History (Pages 3–4): This form gathers important medical history information so we can provide a safe and supportive training environment for your cadet. It is completed by both the applicant and parent/guardian and must be updated as needed after enrollment.
- 🩺Report of Medical Exam (Completed by Doctor) (Pages 5–6): This form records the results of the physical exam required to join the program. It must be completed by the examining physician
- 💊 Medical History Supplemental (If Taking Medication) (Pages 7–8): This form lists any medications your cadet takes. It must be signed and dated by the parent/guardian and, if prescription medications are involved, by a medical professional. If your cadet does not take any medications, simply write “N/A” on the form or use the provided N/A form link
- ♿Request for Accommodation (ADA) (Pages 9–10): – This form is only needed if your cadet requires special accommodations. It must be filled out and signed by the parent or guardian. Requests are reviewed and approved by the Unit Commanding Officer, Regional Director, and National Headquarters. If your cadet does not need accommodations, simply write “N/A” on the form or use the provided N/A form link.
- 👨👩👧 Parental Support Agreement (Page 11): The NSCC is led by dedicated adult volunteers—many of whom are parents. This form highlights the different ways you can get involved and support the unit.
- 🎖️ Uniform and Equipment Responsibility Agreement – Uniform items issued to cadets belong to the Georgia Division and are loaned to your cadet during their time in the program. By signing this form, parents agree to ensure all items are properly returned when the cadet leaves or completes the program.
